LEGAL INFORMATION

I understand the Sevier County Utility District (SCUD) takes this application subject to acceptance. I acknowledge I have read, understand, and agree to all terms of this application. I acknowledge the receipt of a copy of this application for new service.

I hereby declare that the information given on this application is complete and accurate. If there are any changes, I understand that it is my responsibility to notify SCUD immediately with the updated information. In addition, I am authorizing SCUD to obtain a credit report in the connection with this application and for any update, increase, renewal or collection on behalf of SCUD. I understand that SCUD relies on the information in this application and account pay history to make decisions.

Note: It is a federal crime to willingly and deliberately provide incomplete or incorrect information on this application.

 

PREVIOUS APPLICANTS

The undersigned hereby certifies that all past accounts with SCUD have been paid in full whether in the name used on the application, names used on previous applications or prior accounts outstanding in which you resided as either an individual or member of a group using gas service provided by SCUD.

 

CALL BEFORE YOU DIG

You must contact the Tennessee One Call Center at 811 or 1-800-351-1111 three working days prior to digging around gas piping in order for SCUD to properly locate District-owned piping. This is a free service and is required by law.

 

NOTICE

Natural gas is odorless, colorless and non-toxic so SCUD adds an odorant to make it easier to smell for your protection. If you smell natural gas, call SCUD immediately at 865-453-3272 or toll free at 1-888-811-7283. If the odor is strong, evacuate the area immediately, and call SCUD from a safe location. Do not use matches, open flames or other devices that may produce a spark, including turning electrical switches on or off to lights, appliances, computers, etc. Do not re-enter the area until a SCUD representative says it’s safe to return.

 

192.16 CUSTOMER NOTIFICATION -CUSTOMER OWNED PIPING

SCUD is responsible for the main gas line at the road and the gas piping running up to and including the gas meter. The customer is responsible for all gas piping from the outlet of the meter up to and including the gas appliances.

MAINTENANCE

  • The customer is to maintain all piping after the outlet of the meter to prevent corrosion, leaks or damage. Customer owned buried gas piping should be: Periodically inspected for leaks.
  • Periodically inspected for corrosion if piping is metallic. Metallic pipe must be cathodically protected to prevent corrosion. SCUD may be contacted for cathodic protection options if needed. Repaired if any unsafe condition is found.
  • When excavating near buried gas piping, the piping should be located in advance, and the excavation should be done by hand. SCUD is responsible for locating its own lines after TN 811 has been properly notified. The customer is responsible for locating their own lines. The installer, plumber, or heating contractor may be able to assist with locating and repairing the customer’s gas piping.

INSTALLATION

  • No mechanical fittings are allowed underground.
  • Gas piping must not be in contact with any chemicals that may cause corrosion or damage.
  1. Black Iron or Galvanized Pipe – CANNOT be installed underground (even if it is sleeved) or in contact with the ground or standing water.
  2. Copper Tubing – May be installed underground but all underground joints must be silver soldered.
  3. CSST (Corrugated Stainless Steel Tubing):
    • May be used underground if sleeved in PVC or if direct burial CSST is used.
    • The installation must be done in accordance with manufacturer’s procedures.
    • No fittings may be installed underground.
  4. PE-Polyethylene (Plastic) — CANNOT be used above ground.
    • High or medium density PE pipe may be used but must be welded (fused).
    • The welder must be certified to weld which ever type of PE pipe is being used. SCUD is not qualified to weld medium density pipe.
    • Anode-less risers must be used to turn up out of the ground.
    • PE pipe shall not be installed under a building slab or building.

Buried piping must be a minimum of 12 inches deep. See 404.10 IFGC. For exception see 404.10.1 IFGC

Buried gas piping shall not penetrate building foundation walls at any point below grade. Gas piping shall enter and exit a building at a point above grade and the annular space between the pipe and the wall shall be sealed.404.4 IFGC

*Natural gas installations completed prior to June 1, 2014 are grandfathered but black iron and galvanized pipe must be properly cathodically protected if it is to be in contact with the ground. See 404.9, 404.9.1 and 404.9.2 IFGC. The owner is responsible to provide cathodic protection for any steel piping that is underground. SCUD may be contacted for cathodic protection options if needed.

*Propane switch outs are grandfathered, but the owner is responsible to provide cathodic protection for any black iron or galvanized piping that is underground. SCUD may be contacted for cathodic protection options if needed. A hold harmless must be signed for any piping that cannot be inspected.

 

NEW NATURAL GAS INSTALLATIONS CONTRACT

  • A rough-in inspection is required on all gas plumbing and venting that will be concealed or buried.  If plumbing or venting is already concealed, provisions must be made by the homeowner or contractor to expose all concealed plumbing or venting.  We cannot climb on the roof in order to perform inspection.
  • The meter must be set before you call for a final inspection.  A final inspection consists of an inspection of any gas plumbing or venting not seen at the rough-in, a visual inspection of gas appliances and/or equipment, and a pressure test which is done by the contractor or homeowner and witnessed by a Sevier County Utility District Inspector.

All inspections are free and conducted for customer safety.

The homeowner or contractor must provide access to all equipment / appliance.  If needed, access to attic must be provided with a ladder or stairs.  Also, they are responsible for the connection to the meter after the successful completion of the final inspection.  Gas service will not be turned on until the following steps are taken to ensure all installations are safe and installed according to the Gas Code.  Please call the office to schedule your inspection appointment 2-4 days in advance.

Customer knows of no underground obstruction in the path of the proposed natural gas service line, including but not limited to, water, electrical, telephone lines, sewage or septic lines, septic tanks, drain fields, etc.  Customer has disclosed to District all underground obstructions to the best of customer’s knowledge, information and belief.

The undersigned applicant/customer makes application for gas service at the above address and agrees to pay for said services as measured by the District’s meters according to rates applicable.  The applicant/customer agrees to permit authorized agents of the District free access to the premises of the customer for the purpose of inspecting, reading, repairing, or removing property of the District.

Applicant confirms that he will abide by all rules and regulations that are now and may hereafter be prescribed by the said District.  This deposit will be retained by the District as a meter and security deposit until such time that services are discontinued and all gas bills paid.  This deposit is not transferable.

This application is taken subject to acceptance by the Sevier County Utility District.  The District shall have the right, but shall not be obligated, to inspect any installation before service is introduced, or at any later time, and reserves the right to reject any wiring, piping or appliances not in accordance with District’s standards, but such inspection or failure to inspect or reject shall not be regarded as insurance against defects in installation, wiring, piping or appliances and shall not render District liable or responsible for any loss or damage resulting from defects in the installation, wiring, piping or appliances, or from violation of any Ordinance, or Rules and Regulations, now in force or as may hereafter be adopted, or from accidents which may occur upon customer’s premises.  The Applicant acknowledges that he understands that there is a minimum bill charged on each account regardless of the amount of gas used. The Applicant also acknowledges that they have read the Customer Owned Piping Notification information listed above.

The Principal and Co-Signer (if any) bind themselves, jointly and severally, to the District for such sums as Applicant shall be obligated to pay District for gas service and other charges, including but not limited to any necessary collection fees, reasonable attorney fees, and court costs, if any, pursuant to the Policies, Rules and Regulations of the District.
